Telling on myself here. I work for an internist office, and we see a lot of diabetic patients. Unfortunately, some of our diabetic patients will lose fingers, toes, hands, feet, arms, or legs due to diabetic complications. One patient came in, after having surgery to remove most of his right hand. When I brought him in to an exam room to check his BP, I asked him to remove his jacket. He looked like he was struggling a bit, so I asked him (in all innocence):

Me: Need a hand?
Patient: (with a smirk on his face) No pun intended?
Me: (apologizing profusely with a very red face)

Fortunately, he had a pretty good sense of humour about and let me off the hook pretty quickly.
